I've posted a freely downloadable submissions database, created in OpenOffice as an ODB file, for anyone who's interested:

It has forms for easy entry. What it lacks is the ability to create certain reports based on queries. I have an idea what the SQL queries would look like, but am not sure how to use OpenOffice to actually make them happen. If anyone has experience with such a thing and would like to contribute I'd be much obliged. (further details about what sort of queries are also at the link)

Lemme take a look, see what I can do. Although I'll be doing it in LibreOffice ;-) I'll probably have the chance to check it over the weekend... that OK? pm me with an idea of what reports you want.
