Alright, this has probably been asked before.

When I hear a line of text read twice in an episode, is it a recording error or a submission error?  If EA receive a story and a line is typed twice, are they required to read the mistake?

It's a audio editing mistake.  The reader doesn't like the first reading and just reads it again.  The idea is to go back and remove one of the readings, but during the editing not al of the mistakes are removed.  This is why Steve uses a clicker. 

If you listen to the Raw version of EP169, you hear this click after each error.  This gives a visual spike on the recording that makes editing easier.
